Create an Ad hoc Question To add the new question within an existing section Click on the checkbox next to the desired section. If you do not check a section, your question will appear at the bottom of the content page. 3 4 The freeform text box allows you to write out your survey question, or copy and paste it from another document Choose the answer type. In this example we selected Percentage. The survey recipient would respond with the percentage of accurate invoices received in the previous quarter. Notice that the Parent section is mapped to a KPI on the scorecard. Therefore this question is automatically mapped as part of this section. If you are adding a question outside of a mapped section or adding a new section you will need to map those items to the associated scorecard. 3 4 Click Done to save your changes 5 5
